The entrance of 71 Hamilton Street is closed and individuals visiting the Surrogate’s Court need to enter through 77 Hamilton Street.

The case information sheet below is required for all appointments. 

The Surrogate Court only accepts payment in cash, check or money order.

 Please email the following: Surrogate Case Information Sheet (see above), Death Certificate, Last Will and Testament / Proof of Assets, Photo ID of Executor and date of appointment. If we do not receive your paperwork within 24 hours from making your appointment it will be canceled.

If the deceased did not have a will, please provide proof of assets. 

• Home: If you own a home, whether jointly or not: copy of Deed, printed Value of Home (Zillow Estimate) https://www.zillow.com/
• Banks: most recent Bank Statement.  If none available, then list Value as “Inquiry." 
• Vehicle: 1 of the 3 items: Title, Registration or Insurance ID and printed Vehicle Value ( https://www.kbb.com/whats-my-car-worth/ ).

NOTE: On your appointment date, you will need to bring with you original copies of the Application Form and ALL hard copy of supporting documents.
