In order to book an appointment, you must pre-file information about the estate.  Please upload the required documents below.  Once you submit all required documents, you will automatically be directed to our booking page.  Please be patient as the booking page may take time to load. 

The case information sheet below is required for all appointments. 

If the deceased did not have a will, please provide proof of assets. 

• Home: If you own a home, whether jointly or not: copy of Deed, printed Value of Home (Zillow Estimate) https://www.zillow.com/
• Banks: most recent Bank Statement.  If none available, then list Value as “Inquiry." 
• Vehicle: 1 of the 3 items: Title, Registration or Insurance ID and printed Vehicle Value ( https://www.kbb.com/whats-my-car-worth/ ).

NOTE: On your appointment date, you will need to bring with you original copies of the Application Form and ALL hard copy of supporting documents. 

The Surrogate Court only accepts payment in cash, check or money order.
